If you're caring for a loved one living with dementia, you know the journey can be overwhelming. Between doctor's appointments, medications, behavioral changes, and the day-to-day responsibilities of caregiving, many families don't realize there may be additional resources available to help.
One program that is often overlooked is Medicare's GUIDE (Guiding an Improved Dementia Experience) Model.
While this program has been available for some time, many caregivers are still unaware of it. If your loved one has been diagnosed with dementia, it may be worth asking whether they qualify.
What is the GUIDE Model?
The GUIDE Model is a Medicare initiative designed to improve care for individuals living with dementia while providing additional education, coordination, and support for the caregivers who care for them every day.
Depending on eligibility and the participating provider, services may include:
Comprehensive dementia care planning
A dedicated care navigator
Coordination between healthcare providers
Caregiver education and training
24-hour support
Connections to community resources
Qualifying respite services for caregivers
The goal is to help people living with dementia remain safely at home longer while providing families with additional guidance and support. CMS provides these services through participating GUIDE organizations.
Who May Qualify?
According to CMS, individuals generally must:
Have a confirmed diagnosis of dementia.
Be enrolled in Original Medicare (Parts A & B) with Medicare as their primary insurance.
Not be enrolled in a Medicare Advantage plan.
Meet the remaining eligibility requirements established by the GUIDE program.
If your loved one has Medicare Advantage, don't stop asking questions. Many Medicare Advantage plans offer their own care coordination and caregiver support programs, which may differ from the GUIDE Model.

If your loved one has dementia, consider asking:
Does my physician participate in the GUIDE Model?
Is there a GUIDE provider serving our community?
Would my loved one qualify?
What caregiver education is available?
Are respite services available?
What other community resources should we know about?
