What Caregivers Need to Know During Times Like These

With many families home due to weather and changing schedules, it’s a good time to talk about an important topic that affects millions of households — caring for a loved one living with dementia.

Many families don’t realize that Medicare is beginning to expand support for dementia care and family caregivers, especially through a new pilot program known as the GUIDE Model.

Understanding what help is available — and what is not — can make a meaningful difference during an already emotional journey.

🧠 What Is the Medicare GUIDE Model?

The GUIDE Model, which stands for Guiding an Improved Dementia Experience, is a Medicare pilot program created by the Centers for Medicare & Medicaid Services (CMS).

This program is designed specifically to support:

  • People living with dementia

  • Their unpaid family caregivers

Through the GUIDE Model, families may receive:

āœ” Care navigation supportāœ” Ongoing dementia care coordinationāœ” Education and caregiver guidanceāœ” 24/7 support accessāœ” Help connecting to community resourcesāœ” Limited respite support (varies by provider and region)

šŸ‘©ā€āš•ļø Why the Doctor Must Be Involved

To participate in the GUIDE Model:

  • The patient must be a Medicare beneficiary

  • A medical provider must be involved

  • The doctor or healthcare organization must be enrolled as a Medicare GUIDE provider

This means families cannot self-enroll on their own — the care team plays a critical roleĀ in determining eligibility and connecting families to the program.

Not all doctors or healthcare systems participate yet, which is why this support is not available in every state or community at this time.

🩺 What Insurance Is Required?

āœ… Qualifies:

  • Original Medicare (Part A & Part B)

  • Medicare beneficiaries diagnosed with dementia

āš ļø Does NOT automatically qualify:

  • Medicaid alone

  • Private insurance only

  • Long-term care insurance

  • Medicare Supplement plans without Medicare Part B

The GUIDE Model works through Medicare, not private insurance plans.

āŒ What Medicare Does NOT Cover

It’s important for families to understand:

  • Medicare does not pay family caregivers directlyĀ for providing daily care

  • Medicare does not cover full-time custodial care

  • Routine supervision, meal prep, or companionship are not covered under Original Medicare

Medicare may, however, cover skilled servicesĀ when ordered by a physician, such as:

  • Skilled nursing

  • Physical therapy

  • Occupational therapy

  • Speech therapy

🧩 What About Medicare Advantage Plans?

Some Medicare Advantage (Part C)Ā plans may offer additional caregiver-friendly benefits such as:
